In the ancient city of Qichun on the north bank of the Yangtze River, the scent of mugwort has been lingering for thousands of years. This is the hometown of the medical saint Li Shizhen, and also the authentic production area of the "king of mugwort" - Qiai. On this land, an ancient medical technique that uses fire as a medium and mugwort as a material has been passed down from generation to generation and still shines brightly today. It is a representative project of national intangible cultural heritage - "acupuncture and moxibustion (Qichun moxibustion therapy)".
The key figure who brought this ancient technique from the countryside to the national hall and integrated it into modern healthy living through medical books and manuals is Han Shanming, who comes from a family of traditional Chinese medicine in Qichun. Since 1989, he has been practicing acupuncture and moxibustion, and has been rooted in the grassroots for more than 36 years. He not only integrates family moxibustion with classic acupuncture and moxibustion, but also combs the context of moxibustion for thousands of years with his own efforts. After six years and more than 50 times of easy draft, he finally successfully pushed the "Qichun moxibustion therapy" into the national intangible heritage protection list. From writing standardized textbooks, cultivating tens of thousands of moxibustion masters, to visiting more than ten countries with Artemisia argyi, every step of his life embodies the original intention of "using hanging pots to benefit the world and inherit intangible cultural heritage".
In this issue of Ningdian Interview, we specially invite Han Shanming, the national representative inheritor of the national intangible cultural heritage "acupuncture and moxibustion (Qichun Moxibustion Therapy)", to share with you the millennium wisdom and contemporary mission of Qiyi and Qichun Moxibustion.

What are the forms?Han Shanming: Acupuncture and moxibustion are two different external treatment methods in traditional Chinese medicine. Acupuncture, also known as moxibustion therapy, is explained in the "Shuowen Jiezi" as "burning the body to treat diseases". It uses fire as a medium to treat diseases, such as moxibustion. Why are they mentioned together? Because they are representatives of external treatment methods in traditional Chinese medicine.

As the earliest and complete TCM classic, Huangdi Neijing is divided into Lingshu and Suwen, among which Lingshu, also known as Acupuncture Classic, specializes in acupuncture and moxibustion and other external therapies. It sums up the treatment methods of traditional Chinese medicine as "six arts": needle, needle, moxibustion, medicine, guiding and pressing. Before the emergence of metal, people used needle stones to make needles and knives, stimulating the body to treat diseases, known as needle stone therapy. Acupuncture is a method of treating diseases by stimulating acupoints with metal needles; Moxibustion involves burning mugwort to fumigate acupoints; Medicine, which refers to taking traditional Chinese medicine for conditioning; Guidance is the practice of regulating the body through qigong or exercises such as Ba Duan Jin and Tai Chi; Pressing on stilts is similar to massaging bones.

The traditional Chinese medicine treatment system is divided into two categories: internal treatment and external treatment. Internal treatment mainly refers to drug therapy, which has gradually emerged since the Han Dynasty. Zhang Zhongjing is an iconic figure. Prior to this, drug therapy often used single herbs, while Zhang Zhongjing pioneered the system of syndrome differentiation and treatment. "Treatise on Cold Damage" contains more than 100 prescriptions that enable drugs to work together and exert their effects. In addition to "medicine", the other five arts belong to external treatment, and acupuncture and moxibustion is a typical representative of them. Acupuncture and moxibustion has a long history. It is said that Fuxi invented acupuncture and moxibustion during the period of the Three Emperors and Five Emperors. The Yellow Emperor's Inner Canon during the Warring States period detailed the meridians, acupoints, needles, needling techniques, treatment principles, as well as the origin, indications, and principles of acupuncture and moxibustion. The ancients believed that 'what needles do not do, moxibustion is appropriate', emphasizing that the two have their own expertise and complement each other to enhance efficiency.

For thousands of years, acupuncture and moxibustion have been important treatment methods in traditional Chinese medicine clinical practice. In recent decades, they have become a key carrier for the international dissemination of traditional Chinese medicine. Today, acupuncture and moxibustion has been legally recognized in many countries and gradually incorporated into the local medical system, becoming an important bridge for health exchanges between China and foreign countries. This not only reflects the universal value of acupuncture and moxibustion therapy itself, but also reflects the vitality of TCM culture across time and space.

How is "moxibustion" combined and emphasized?Han Shanming: In clinical practice, "acupuncture" and "moxibustion" are complementary and synergistically used. I come from a family of traditional Chinese medicine practitioners. Although my grandfather and father were farmers, they relied on their ancestral medical skills to diagnose and treat diseases and solve problems for their neighbors for many years. I have been following my parents since childhood to collect herbs and make medicines, observing them using methods such as moxibustion and acupuncture to treat diseases, and gradually putting them into practice. This environment has given me a deeper understanding of traditional Chinese medicine.

In my nearly forty year medical career, acupuncture, moxibustion, and traditional Chinese medicine have been the most commonly used methods. Especially moxibustion therapy, its clinical effects often exceed what is learned in textbooks. Based on observations over the past ten to fifteen years, from the perspective of traditional Chinese medicine, modern people have two main problems with their physical constitution: firstly, insufficient yang energy, which is often caused by staying up late, overwork, cold eating, irregular sleep patterns, and environmental and climatic influences; Secondly, there is a strong emphasis on dampness and evil. The accumulation of dampness into phlegm and the coagulation of phlegm into lumps are the pathological basis of many common contemporary diseases, such as obesity and tumors. However, the fundamental reason for the difficulty in transforming dampness is still the lack of yang qi and the inability to warm water and dampness. Therefore, the characteristics of moxibustion therapy, such as warming and tonifying yang, dispelling dampness and dispersing lumps, are particularly in line with the physical characteristics of modern people. In recent years, with the accumulation of diagnosis and treatment experience, we have increasingly relied on moxibustion as the main treatment method for difficult diseases such as rheumatoid arthritis, ankylosing spondylitis, some eye diseases, and Sjogren's syndrome, and have achieved good feedback. After the epidemic, moxibustion has become increasingly widely recognized and used among the public.

For each patient, whether to use acupuncture as the main treatment, moxibustion as the main treatment, or to combine with traditional Chinese medicine, still needs to be determined based on the differentiation of symptoms. When facing complex illnesses, I will combine moxibustion, acupuncture, and traditional Chinese medicine for oral administration. Single therapy can sometimes be difficult to achieve ideal results. When multiple treatment methods are combined, such as teamwork, each can develop its own strengths and complement each other, in order to better integrate advantages and make up for limitations. This is also a reflection of the holistic treatment thinking of traditional Chinese medicine.

Han Shanming: I come from a traditional Chinese medicine family. According to research, my ancestor Han Tai was an imperial physician in the mid Ming Dynasty's Chenghua period. Since the Ming Dynasty's Chenghua period, I have been the 17th generation descendant. As someone who has been immersed in the atmosphere of traditional Chinese medicine since childhood, I resolutely chose to major in traditional Chinese medicine after the college entrance examination. At first, my father didn't understand. He believes that his family has been practicing medicine for many years without achieving great success, and it is not easy to get into university. Therefore, he should strive for a broader horizon. In the end, with the support and mediation of my grandfather, my choice was respected. In 1984, I was admitted to Hubei College of Traditional Chinese Medicine to study acupuncture and moxibustion. After graduation, I chose to return to my hometown Qichun. On the one hand, the traditional Chinese medicine heritage in my family makes it difficult for me to let go; On the other hand, as the hometown of the medical saint Li Shizhen and a practitioner of traditional Chinese medicine, I deeply feel the responsibility of inheriting family traditions and promoting Li Shizhen's traditional Chinese medicine culture in Qichun. Since then, I have devoted myself to the clinical and inheritance work of Chinese medicine acupuncture and moxibustion.

In 2013, Qichun County Cultural Center planned to declare the project including Qichun moxibustion therapy as a provincial intangible cultural heritage. As the first undergraduate majoring in acupuncture and moxibustion since the founding of the People's Republic of China in Qichun County, I have been committed to the clinical work of acupuncture and moxibustion for a long time. Although my daily affairs are busy, when I learned that Qichun moxibustion therapy is expected to apply for the World Heritage, I and the leaders of the hospital felt that it was of great significance. We unanimously believe that this is not only the protection of a traditional skill, but also an important means of systematically excavating the rich cultural history of traditional Chinese medicine in the hometown of the medical saint Li Shizhen, achieving protection and inheritance. Therefore, we immediately decided to fully cooperate with the application work, hoping to take this as an opportunity to let Qichun moxibustion therapy go global and radiate new vitality under the protection framework of intangible cultural heritage.

The moxibustion therapy in Qichun area has a long history and is widely used. The local area is rich in high-quality Artemisia argyi, and medical experts such as Li Shizhen have extensively recorded various moxibustion therapies in the Compendium of Materia Medica. In addition, there are many moxibustion methods with local characteristics circulating among the people in Qichun. So we have carried out a lot of basic work, including sorting out relevant materials, collecting ancient books and documents (especially the Compendium of Materia Medica with nearly two million words), and going deep into the folk to conduct interviews and research, and carrying out practical demonstration of folk therapy, taking its essence to remove its dross. In the end, the project was successfully included in the fourth batch of provincial intangible cultural heritage representative projects in Hubei Province.

In 2014, coinciding with the launch of the fourth batch of national representative projects for intangible cultural heritage, we attempted to apply for the national list on the basis of provincial-level projects. However, due to time constraints and insufficient preparation, our project was unable to meet the higher requirements of national intangible cultural heritage standards. But after being rejected this time, we did not give up, instead we calmed down and continued to deepen our research. The application for intangible cultural heritage requires a clear historical origin and continuous inheritance of the project, usually lasting for more than a hundred years or at least three generations. The Qichun moxibustion therapy has been passed down for over a hundred years with distinct generations. To fully demonstrate its historical continuity and cultural depth, we have conducted detailed research and summarization of relevant records from various dynasties. Due to the difficulty in finding relevant information before the Tang Dynasty, we started researching from the Tang Dynasty and went through the Song Dynasty, Ming Dynasty, Qing Dynasty, and modern and contemporary times to systematically sort out the inheritance of Qichun moxibustion therapy.

On this basis, we strictly followed the requirements of the national intangible cultural heritage application form, conducted extensive field investigations and ancient book collection work, and combined with the actual application of this therapy in traditional Chinese medicine clinical and health fields, we extracted three core advantages of Qichun moxibustion therapy: firstly, the moxibustion material is authentic and has strong thermal penetration ability; Secondly, the technique is unique and the moxibustion points are precise; Thirdly, it is simple, reliable, and widely effective. At the same time, we summarized the three representative moxibustion methods in the Qichun moxibustion therapy: the Lei Huo Shen Needle moxibustion method initiated by Li Shizhen, the Qichun big moxibustion method contained in the Compendium of Materia Medica and inherited in the folk, and the Qichun fire moxibustion method. In 2020, in combination with the prevention and control practice of COVID-19, we further summarized the four epidemic prevention methods of Qichun moxibustion (such as the epidemic prevention method of Qichun fumigation moxibustion, the epidemic prevention method of Qichun steam moxibustion, the epidemic prevention method of Qichun warm moxibustion, etc.), which together constitute the core skill system of Qichun moxibustion therapy.

In the following years, we continued to improve our application materials and extensively invited experts and scholars from both within and outside the province, as well as at the national level, to provide guidance. The application underwent over fifty carefully crafted drafts before and after submission. After years of unremitting persistence, Qichun Moxibustion Therapy was successfully selected as a representative project of the fifth batch of national intangible cultural heritage in 2021. It is worth mentioning that this is currently the only national intangible cultural heritage project selected for moxibustion therapy in China. The difficulty of applying for national level intangible cultural heritage of traditional Chinese medicine is extremely high. Taking Hubei Province as an example, although it has a long history of traditional Chinese medicine culture and abundant resources, as of the release of the fifth batch of national level intangible cultural heritage list, there are only 6 traditional Chinese medicine projects, among which the only one directly related to disease treatment is Qichun moxibustion therapy. The other projects, such as the Han style Peng Yin Ting traditional Chinese medicine processing techniques, Ma Ying Long eye medicine production techniques, Xia Shi Dan medicine production techniques, and Zhen Shi rheumatism Ma Qian Zi therapy, are mostly weakly related to clinical diagnosis and treatment; The Qichun moxibustion therapy requires direct application to the human body to achieve diagnostic and therapeutic purposes, so its application standards are also stricter.

Han Shanming: As the saying goes, "Good moxibustion materials make moxibustion better." High quality moxibustion materials are the prerequisite for achieving good moxibustion treatment effects. As a plant-based medicinal herb, Artemisia argyi has a deep historical and cultural connection with the Chinese nation. It has been recorded since the time of the Book of Songs and is also commonly found in medical books, cultural relics, and works throughout history. The growth of plants is closely related to the environment, just as "oranges grown in Huainan are called oranges, and those grown in Huaibei are called aurantii". In the field of traditional Chinese medicine, this phenomenon is referred to as "authentic", which has led to the concept of "authentic medicinal herbs", referring to medicinal herbs that grow in specific regions, have excellent quality, mature technology, and are widely recognized as high-quality in the industry.

Before the Song Dynasty, there was no special emphasis on the production areas of Artemisia argyi. However, areas such as Qichun in Hubei, Nanyang in Henan, Yueyang in Hunan, Chuanai in Shandong, and Sichuan could all be used for medical practice. After extensive research, the earliest clear record of the Ai Cao Dao real estate area can be found in the book "Compendium of Materia Medica Products" edited by Liu Wentai in 1505 AD. The book first mentioned the Ai Cao Dao real estate area as Qizhou (now Qichun County, Hubei Province) and Mingzhou (now Ningbo, Zhejiang Province). Siming Mountain in Ningbo, Zhejiang once produced mugwort called sea mugwort. Therefore, as a representative of authentic mugwort, the status of "Qiai" has continued to this day and has been confirmed in the Pharmacopoeia of the People's Republic of China.

The authentic advantages of Qiai are mainly reflected in the following five aspects:
Firstly, it has outstanding shape and high velvet yield. Qiai plants are tall and robust, with an average height of 1.8-2.4 meters and a maximum height of over 3 meters. The leaves are large and thick. On the same area of land, the amount of Artemisia argyi produced is higher and the utilization rate is higher.
Secondly, it has sufficient heat and strong penetration. The Compendium of Materia Medica records that "moxibustion cannot penetrate the wine jar in other places, while moxibustion with Artemisia argyi can penetrate directly and thoroughly, which is different." This means that when Artemisia argyi is ignited for moxibustion, it is easier to penetrate the skin and meridians. Modern experimental verification shows that the short infrared radiation released by the combustion of Artemisia argyi is more suitable for human absorption, and its combustion value is about 20% to 25% higher than that of other Artemisia argyi plants.
Thirdly, it has a high content of essential oils. The essential oil content of Artemisia argyi can reach 1.3% -1.5%, while the essential oil content of Artemisia argyi in other places is 0.3% -0.5%.
Fourthly, flavonoids and other active ingredients are abundant. The content of flavonoids (total flavonoids) in Artemisia argyi is 1 to 4 times that of ordinary Artemisia argyi. Flavonoids have multiple biological activities such as anti-inflammatory, antioxidant, and immune regulation, and are one of the important material foundations for the therapeutic effects of Artemisia argyi leaves.
Fifthly, it has been highly praised by medical practitioners throughout history. From the Tang Dynasty to contemporary times, Artemisia argyi has long been regarded as the preferred moxibustion material in history, and its authentic status has been continuously recorded in writing and verified by clinical practice.

This authentic advantage directly translates into unique moxibustion effects: high-quality moxa velvet ensures the basic efficacy of moxibustion treatment, sufficient heat and strong penetration can more accurately act on lesions, and rich essential oils and total flavonoids enhance the pharmacological effects of warming meridians, dispelling cold and relieving pain, making Qichun moxibustion therapy achieve the unique value of "precise moxibustion application and wide efficacy" in clinical practice, which is also an important foundation for it to become a national intangible cultural heritage.

Han Shanming: The development of Qichun moxibustion therapy cannot be achieved without the dual efforts of building a solid industrial foundation and exploring innovative paths. Since the 1990s, Qichun County has been exploring the path of empowering county development with medical culture. Since 2013, Qichun County has established a development path based on Qiyi, one of the "Four Treasures of Qichun", as the core raw material, and supported by the national intangible cultural heritage project "Qichun Moxibustion Therapy". After years of cultivation, a full industry chain covering planting, processing, product research and development, and health services has been formed. There are nearly 4000 related enterprises in the county, with a planting area of about 200000 acres of Artemisia argyi and a comprehensive output value of over 16 billion yuan. The brand value exceeds 14 billion yuan, laying a solid industrial foundation for the inheritance and development of Qichun moxibustion therapy.

In this industry pattern, as inheritors of intangible cultural heritage and practitioners of traditional Chinese medicine, we actively play a professional role, leverage the credibility of intangible cultural heritage projects, and promote industry norms and talent development. In the early stage of industrial development, product homogenization was prominent, and the market's understanding and application of moxibustion technology were insufficient. Therefore, since 2013, we have been systematically carrying out the construction of a training system for moxibustion therapists, including the development of training content and assessment standards, and promoting their gradual professionalization and standardization towards innovative development.

I have presided over the compilation of a series of training materials such as "Qiai Health Moxibustion Therapy". Among them, "Qiai Health Moxibustion Therapy" was selected as a training material for scarce skilled talents in Hubei Province in 2018 and is still an important basis for the training and assessment of health moxibustion therapists. The textbook fully embodies the characteristics of Qichun moxibustion therapy and incorporates various local folk moxibustion methods and contemporary inheritors' moxibustion clinical techniques. At present, more than 20 training schools have been established in the county, with a total of over 80000 Qichun moxibustion masters trained. The entire industry chain has driven employment for about 300000 people, effectively promoting the active inheritance and widespread dissemination of intangible cultural heritage skills.

Our inheritance work has been widely recognized nationwide, but there is still room for improvement. The core breakthrough point for further development in the future lies in the deep integration of traditional technology and modern technology. We are currently collaborating with research institutions, universities, and high-tech enterprises to explore the application of AI technology, moxibustion robots, and other technologies in the field of moxibustion. Our goal is to liberate practitioners from heavy manual operations, reduce service costs, and enable more people to enjoy affordable and high-quality moxibustion services, thereby contributing to the construction of a healthy China.

In a 2016 exhibition event in Almaty, Kazakhstan, a local child of about four to five years old experienced moxibustion under the guidance of his relatives (local cultural department officials) due to long-term loss of appetite. We apply moxibustion to it on a simple treatment bed, selecting acupoints such as Spleen Shu and Stomach Shu on the back, and using a portable moxibustion box for operation. After about twenty minutes of moxibustion, the child suddenly felt hungry and wanted to eat, which was unbelievable to those around him. This rapid effect also caused a great response in the local area. After the news spread, it attracted residents from hundreds of kilometers around to come and experience it. After the daily activity ended, there were still many people waiting, forcing us to extend the service time.

In 2018, I visited Italy, Hungary, and the Czech Republic with the delegation of "Jingchu Culture - Telling the Story of Intangible Cultural Heritage along the Yangtze River". At a special exhibition at the Budapest National Museum in Hungary, former Italian Prime Minister (also a Chinese medicine enthusiast) came to visit. We introduced him to the medicinal value of Artemisia argyi and the history of moxibustion recorded by Li Shizhen. He showed a strong interest and shared his experience of receiving moxibustion treatment in Budapest.

At the same event, a Hungarian botanist in his 60s initially mistook the mugwort we were carrying for a cigar, but after explanation, he remained skeptical. After taking his pulse, I clearly pointed out that there were old injuries on his body. He was very surprised and explained that when he was in his twenties, he fell while skiing and suffered damage to his left hip joint. At that time, there was no obvious pain, but as he grew older this year, he often woke up in the middle of the night with pain. We immediately performed acupuncture on it and applied Qiai fever moxibustion patches. The next day, he specifically thanked us. After the moxibustion treatment that night, not only did the pain greatly ease, but his sleep also improved. For this reason, he specially gifted me a handwritten book titled "Flora of the Carpathian Mountains", and I also gave back a product of Artemisia argyi as a souvenir.

There are many stories like this. Every time I see moxibustion therapy recognized and trusted by people overseas, a sense of pride and mission arises within me. These experiences also make us firmly believe that Qichun moxibustion, with its unique therapeutic effects and cultural connotations, will shine even brighter in international cultural exchanges of traditional Chinese medicine.

Han Shanming: In fact, moxibustion, in the form of fire therapy, is not unique to the Han ethnic group. Many ethnic groups have a tradition of using warmth to treat diseases in their history. Therefore, the overseas public did not encounter difficulties in accepting it as expected, but rather out of curiosity and a sense of surprise and recognition after experiencing its effects firsthand.

To promote the "going global" of Qichun moxibustion, priority needs to be given to promoting standardization and normalization construction. In recent years, systematic work has been carried out at both the local and national levels: Qichun County and Hubei Province have formulated standards related to Qiyi products; National institutions such as the Chinese Academy of Traditional Chinese Medicine are also promoting the standardization of moxibustion techniques; At the same time, the assessment standards for international acupuncture and moxibustion and moxibustion therapists have gradually improved. These efforts have laid the technical and management foundation for the international dissemination of moxibustion.

In the past decade, especially with the revival of traditional Chinese medicine at home and abroad, moxibustion therapy has not only been widely recognized domestically, but also increasingly received attention and welcome internationally. On this basis, further improving these standard systems and connecting them with overseas is the key to the smooth integration of Qichun moxibustion into different medical systems and cultural environments.

Han Shanming: A few days ago, a German team came to Qichun for inspection and showed a strong interest in my book "Qiai Health Moxibustion Therapy". Currently, we are negotiating with the publishing house to translate it into a German version for promotion in Europe, which is a recent development in foreign exchanges. I have the following plans and ideas:
Firstly, cultivating an inheritance team is crucial. I am already over sixty years old and can work for another ten or twenty years, but this is limited in terms of project inheritance time. Traditional Chinese medicine has been passed down for thousands of years, relying on the relay of generations. Therefore, I want to lead a team with strong inheritance ability, solid professional knowledge, and a willingness to inherit Qichun moxibustion therapy, to ensure the continuity of inheritance work from generation to generation. These inheritors must have an international perspective and not be limited to local areas. They should be trained comprehensively from various aspects such as professional skills and external communication skills.

Secondly, it is urgent to develop compatible products. Traditional moxibustion involves open flames and smoke, and is strictly restricted in many regions abroad. Therefore, how to develop moxibustion products suitable for local application scenarios while retaining the essence of tradition is a major issue that we must overcome.

Finally, being proactive in "going out" is indispensable. As national level inheritors, we shoulder the responsibility of representing the country and telling the story of China and traditional Chinese medicine well. The inheritance and dissemination of moxibustion therapy aims to spread China's unique cultural ideas, namely "using medicine to convey the message". This requires us to constantly learn, improve our skills, and enhance our cultural literacy, in order to better showcase the profound heritage and contemporary value of traditional Chinese medicine on the international stage.
